Below are the forms. Please make sure that you fill out the correct form if you are a delegate, non-delegate or a non-member is attending with you.
In addition, please make sure that the form is being sent to the correct person. The Delegate Registration Form is sent to the Executive Secretary, Beth Wallis. All other forms are sent to the Convention Coordinator. June 12 is the deadline for the Delegate Registration Form and for the tours! There are always some mailed incorrectly each year. Read at the bottom of the page the directions or feel free to email Michelle at dttmgreenwell@gmail.com if you have questions. Forms can always be emailed and checks mailed separately to save on postage.
